Collaborative Management Program (CMP)

Establishment of CMP
In 2004, the State of Colorado established CMP (House Bill 1451), providing funding for counties to voluntarily facilitate and coordinate services for multi-system involved youth. The state provides funding for CMP through the local county Department of Human Services.

CMP in Eagle County
The Eagle County Department of Human Services sub-contracts the Eagle River Youth Coalition (ERYC) to manage CMP, including the hiring and supervision of the CMP Coordinator. Working closely with an Interagency Oversight Group (IOG), the CMP Coordinator is responsible for implementation of CMP.

The ISST - CMP Service Model for Coordination
CMP utilizes Individualized Service and Support Team (ISST) meetings to ensure collaboration of services for multi-system involved youth. ISSTs bring the youth, the family and providers to the table to work together to develop a service plan to address the youth and family concerns.
The ISST is a family-driven meeting where family members decide what services and resources they want to access or participate in that will ensure better outcomes for the youth facing the challenges.

Youth Referrals for ISST Meetings
Some of the Eagle County IOG partners facilitate their own ISST meetings as part of the CMP. Other ISSTs are facilitated by the CMP Coordinator or other neutral facilitator. To qualify for an ISST:
A referral must be submitted by one of the IOG partners.
Youth being referred must be multi-system involved.
Youth must be 0-18 years old.
Youth must reside in Eagle County.

Eagle County IOG Partners – the IOG provides oversight much like a board of directors. The Eagle County IOG meets quarterly to review program and budget reports. They also develop short and long-term CMP goals and provide continuous quality improvement.
